As teachers we all the importance of good documentation, but why is it so hard?  It seems every year I plan to do a better job documenting student behavior, incidents, etc. and every year I fail.  I cannot find a way to document that is quick, organized, and easy to refer back to if needed.  Can any of you let me in on your method of documenting that has worked for you?

Well I prefer to write all that I have observed on a paper and then add it  on an Excel sheet. This helps me to keep a track of the students regular performance and not miss on to any observation. In the end I have a detailed received of each and every student whenever it is required. It is difficult to remember the observations, so writing it down and then feeding it always helps. Then there are also softwares available for the same which help you make the job easier.

It seems to me that I've seen an app for that.  I looked but I can't seem to find the one I was looking for, but here is a free app I stumbled upon: http://itunes.apple.com/us/app/teacherpal/id389584618?mt=8.  It is called TeacherPal.  It's free, so if you have access to that type of technology, it might be worth it.
