I would like to design a peer review feedback fomr as a simple word docment.

Is their any guides/articles.tutorials/advice which provides good practice/techniques etc on creating good peer review feedback forms for students to critique each others work?

I think you need to begin with a rubric for the activity. If it is clear and simple (most are not) then the students can use it as a checklist for their peers.
Sorry this might sound dumb, but what do you mean by the term rubric?

The activity is students critiqing each others early powerpoint protype design, focussing on usability.
I don't use Word and run prints for peer evaluations. Because I'm in a lab, I use online tools. I can share with you the steps I take when setting up peer evaluations...I incorporate this frequently in both small and large projects.

1) After a project, student work is posted either a) in our shared drive, b) in their Google Sites, c) on my Word Press site, or d) in my YouTube channel. Students can then access each others' product for peer evaluation.
2) I create a Google Form for the product which contains the rubric for scoring.
3) In my Word Press site (my teacher site), I create a link to the products and a link to the Google Form. Students are directed to assess, alphabetically, the three or four products that follow theirs.

thanks for your help, yeah i would love to see your google forms etc.

The pilot module is on the institions WebCT, from there students can upload their products i think.

I will consider your ideas on an on-line form. How do students access these complete forms again?

It would be great if the WebCT has a feature with which i can create an online form. This way there is no complication with external sites.
