Google Forms

I recently used Google Forms to do a pre- and post- assessment. Groups of 8 students completed the assessment on the class computers and upon submission, clicked the link to return to an empty form for the next student. Only 48 responses were recorded and it wasn't until late afternoon that an error message alerted us to the problem. Has anyone else had this problem? How do you avoid or fix it?
