Individual Learning Plans - Maths and Science

I am really struggling to write meaningful Individual Learning Plans for Year 6/7 students in maths and science. Can anyone recommend good resources that have specific descriptors suitable for students with disabilities? I have found lots of information about what should be happening in schools, but very little practical advice for teachers in the classroom.
