Online Education Survey: need help generating questions

Hi all - I have been teaching online middle school for 4 years now and am developing a workshop for teachers and schools who are starting to look at how to implement online education. 

I would like to set up a pre-survey before going into the school to get a good feel for where the teachers and administration is at and what their knowledge and openness is to online eduction. 

I know some of you are in a brick and mortar school and am wondering if you have some ideas for questions to ask?   Any and all feedback appreciated :-)
