Teacher/Teaching Assistant Evaluation tool Android

I'm looking for a free teacher evaluation tool to use on my android tablet or creative ways I can create one using Microsoft word and a different program (Google docs) (I've tried quick office and document wiz) but some of the things I want on my form aren't being read in those programs. I'd also be willing to buy a cheap app, just not looking for a yearly subscription service or a $300 fee which is what I'm coming across on the internet. Any ideas would be helpful.. Thanks
