Web 2.0 Release Forms/Acceptable Use

As the school year begins and I am getting ready to use technology to increase learning in my classroom, I realize my district's student information release form is not adequate for the technology I am using in my classroom.  I use blogs, wikis, glogs, voicethreads and other web 2.0 tools with my fourth graders.  What type of release forms do you use with your parents?  I am planning on creating one and would love to see examples or hear ideas about what tech savvy school districts/classrooms use.
