Hi.  Does anyone use a form that a teacher would fill out when requesting technology in their classroom?  We have teachers wanting SMART boards, or projectors or document camera but don't have a form for these hardware request.

Some things that I would want on it would be:  how will this impact student learning? what type of professional development would you need? etc.

A big concern when requesting new hardware in schools is price, but there's a TON out there that teachers can get for little or no cost. My blog might help you get some ideas for cheap/free ways to integrate technology into the classroom, specifically the post about getting a $55 interactive whiteboard.
A form 1.0 ? You're kidding, right? What about just sending an IM? It is faster, more efficient and to the point. Or couldn't tchrs just sign up for limited supplies on a google calendar - first come, first serve. Leaves you out of the scheduling and only having to deliver (unless you get a student aide to do that for you)
I wish I could use google calendar or IM but all those features are blocked at our district. Right now the only thing the IT dept will allow is 1.0.
